The innovative platform is designed to address critical social determinants of health, which are nonmedical risk factors, such as life circumstances and environments, that can affect a person’s health and well-being.

Floridians using the CarePlus Community Navigator will be able to find and connect to valuable local resources, such as food banks and transportation and affordable housing options. Types of services listed are based on those most requested in the areas where they live. Initial utilization shows that 47% of searches on the CarePlus Community Navigator are related to food security, 26% to housing, and 8% to transportation.
